In consideration of being permitted to participate in knife making activities at AZ Custom Knives Ltd ("AZCK"), I acknowledge and agree to the following terms.
Activities include grinding, shaping, drilling, heat treating, handle fitting, and finishing of knives and bladed tools using power tools, hand tools, and related equipment. Participants may be exposed to sharp edges, rotating machinery, hot materials, metal dust, noise, and other workshop hazards.
I understand that knife making involves inherent risks of injury including cuts, burns, eye injury, hearing damage, and other bodily harm. I voluntarily assume all risks associated with my participation.
I release, waive, and discharge AZCK, its owners, employees, and agents from any and all liability, claims, demands, and causes of action arising out of or related to any loss, damage, or injury that may be sustained during my participation.
I confirm that I am physically and mentally fit to participate in the activities described and that I have disclosed any relevant medical conditions to AZCK prior to participation.
This waiver is governed by the laws of the Province of Alberta, Canada.
If the participant is under 18, a parent or legal guardian must also sign below.
1. Acknowledgment of Risk: I confirm I understand the specific risks — sharp tools, hot materials, heavy equipment, rotating machinery — and consent to the minor's participation.
2. Indemnity: I agree to indemnify AZCK against any claims arising from the minor's participation.
